This is one very-well-known setting of the Agnus Dei.
It is from the Kyriale, which is a book with 18 Gregorian chant settings for the parts of the Mass.
Because it is from the 18th setting in the book, it is know as as the Agnus Dei 18 or Angus Dei XVIII - although many people just call it "the plainchant Agnus Dei".
