This is a simple template or worksheet for organising a church service.

It does not include any specific elements (readings, hymns, prayers, communion, call-to-worship, etc), so can be used in any denomination or type of service. 

Using a planning form like this helps to understand and communicate:

• What exactly is happening in the service
• In what order
• Who is doing what, and
• (for music) What key it is to be played in. 

The form is one page long - so you can see all the elements in the service at a glance, but has enough space to list every single item in most cases.
